Location : Middletown, NY
Hours : M-F 40 hours weekly
Salary : $70,000 to $75,000
Position Summary :
The Provider Relations Director reports directly to the Chief Program Officer (CPO) and plays a vital role in ensuring excellent customer service for community-based provider agencies that refer individuals to our organization for services. This individual will be responsible for cultivating strong partnerships, ensuring alignment with organizational goals, and supporting programs in delivering comprehensive services to meet the needs of individuals and families.
Key Responsibilities :
Provider Agency Engagement :
- Build and maintain relationships with community-based provider agencies offering Mental Health, Substance Use Disorders (SUD), Intellectual and Developmental Disabilities (IDD), Housing, Self-Directed Services, Department of Health (DOH) Services, and Employment Services.
- Act as the point of contact for provider agencies, ensuring open and effective communication and alignment with organizational priorities.

Qualifications :
Bachelor’s degree in Social Work, Public Administration, Healthcare Management, or a related field (Master’s degree preferred).
Minimum of 5 years of experience in provider relations, community engagement, or program management within the nonprofit or human services sector.
Proven knowledge of community-based service delivery systems, particularly for Mental Health, SUD, IDD, DOH, Housing, and Employment.
Strong relationship-building, communication, and problem-solving skills.
Experience working with diverse stakeholders, including government agencies, provider networks, and community groups.
Ability to travel regularly between Hudson Valley and NYC.
