What are the responsibilities and job description for the ECM Care Coordinator position at Access TLC Health Care?
Access TLC is currently seeking for a Full-time Care Coordinator to join our team. The Care Coordinator provides case management and regular field visits to patients.
Access TLC support patients by arranging for community-based and other services that allow recipients to experience an enhanced and enriched quality of life. The right candidate will be eager to learn, compassionate, kind and a genuine team player.
Position Responsibilities:
- Conducting assessments, developing care plans, arranging for and monitoring service delivery, maintaining progress notes and case records, conducting quality assurance reviews, and collecting data.
- Makes visits to patients to establish ongoing eligibility for the program, to monitor the safety and services identified in the Individual Service Plan and address the concerns of the patient.
Requirements Include:
- Must have either a Bachelor or Masters Degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology, Sociology, Health or Human Services or related field.
- Must have a clean California Driver’s License with a vehicle that is insured in accordance with state/or organization requirements
- Must be willing to make patients visits out in the field.
Job Type: Full-time Monday through Friday 8:30 am – 5:00 pm
Rate of Pay: $22.00 /hour
