Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.
About the Role:
We are looking for a proactive employee to act as a facilitator for CDPAP services between consumer and PPL and manage the day-to-day operations of the Consumer Directed Personal Services Program.
Responsibilities include:
- Assist current and new consumers, and Personal Assistants (PA), register for CDPAP services
- Assist consumers and PAs transition to services through PPL
- Monitor EVV compliance and overtime usage and follow up with consumer and PA if issues are noted
- Maintain database input for all demographic and program data. Updates demographic information when changes occur
- Provide consumer onboarding and training
- Provide consumers with EVV initial and annual EVV training
- Train participants and designated representatives in supervisory and problem solving skills; assist them to manage issues with their PAs and monitor their PAs healthcare requirements and completing attendance processes accurately and timely
- Report consumer incidents and allegations of neglect /abuse to PPL
- Provide guidance and support to consumer in all employment activities as appropriate
- Assist with PA onboarding and training
- Provide person-centered, quality, services
- Maintain compliance with applicable program rules and regulations
- Ensure compliance with federal, state, and local employment laws and regulations, as well as with company policies
- Perform Other Duties as Assigned
Education & Experience
High school diploma/GED, required. Associate degree, preferred. One year experience in customer service or working with people with disabilities, preferred.
Conditions of Employment
Must have a valid NYS drivers license in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs.
