SCHEDULE:
Full-Time, Monday – Friday, 8:00 AM – 5:00 PM;
Rotating Saturdays (8a – 12p) / Weekend availability
PAY RANGE:
$16.00 - $19.00

Job Summary
The Postpartum Community Health Worker (P-CHW) for AccessHealth will spearhead AccessHealth's new program trying to prevent unintended pregnancies in women that have just given birth. The P-CHW will maintain a caseload of new moms that bring their babies to the AccessHealth pediatric department. During scheduled well-child visits, the P-CHW will take time to sit with the new moms, find out how they're doing and whether they have contraception plans. Using One Key Question and similar evidence-based techniques, the P-CHW will aim to ensure that new moms at AccessHealth make informed choices about their reproductive future. The P–CHW will provide individualized home visiting services and clinic-based screening from 0 to 6 months postpartum to an assigned caseload. The P-CHW for AccessHealth is a frontline public health worker who serves as a member of a collaborative care team that aims to improve maternal health outcomes through a medical home-based model.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
- Screens all referrals received from internal or outside entities to determine program eligibility.
- Provide curriculum-based maternal health education to parents with an enrolled pediatric patient.
- Prepare contraceptive education and provide culturally competent postpartum education.
- Assist assigned participants with accessing resources, including, but not limited to food, health insurance, housing, quality care, transportation, and health information.
- Perform screening for postpartum depression on all eligible participants.
- Complete home-based visits and follow-up with assigned participants to ensure access to medical care, nutritional food, and appropriate medicine management.
- Link participants with appropriate resources and referrals to outside agencies.
- Assesses needs of participants and perform intakes.
- Participates in department meetings and appropriate meetings with outside entities as determined by management.
- Properly documents activities performed and assistance provided in the participant’s chart in a timely manner.
- Prepares weekly participant reports and maintains caseload of eligible participants.
- Updates management and others as appropriate of any changes to caseload or participant’s enrollment status.
- Contacts participants between visits to follow up on status and confirm upcoming appointments.
- Assist enrolled participants in applying for Medicaid, CHIP, and CHIP Perinatal programs, PHC, SNAP and Temporary Assistance to Needy Families.
- Develop a system for determining eligibility expiration for the enrolled participants, assist current participants to continue their eligibility, and initiate registration process when needed.
- Accepts job related training and development in new tasks and technologies.
- Performs any additional home visits as deemed necessary by management.
- Comply with all company policies, procedures and regulatory requirements.
- Performs all other duties as assigned by management.
Qualifications
SKILLS AND EXPERIENCE:
- Spanish Bilingual ability REQUIRED.
- Two years of experience in customer service, care coordination, or case management.
- Experience documenting and navigating an Electronic Health Record (EHR) system.
- Ability to operate a computer, fax machine, and scanner.
- Good communication skills.
- Ability to work with people of all socioeconomic levels.
- Ability to communicate orally and in writing with good organizational skills.
- Be a consumer advocate for quality care and services.
- Reliable transportation required and experience/ comfort with home visits preferred.
EDUCATION/LICENSURE/CERTIFICATION QUALIFICATIONS
- High School Diploma or GED.
- Certified Community Health Worker or two years working in a community health center.
- Other experience may be accepted in lieu of Education requirements at the discretion of management.
