The Home Health Scheduler is a remote position responsible for coordinating and maintaining the schedules of home health nurses and caregivers to ensure continuous and quality patient care. This role acts as a liaison between patients, field staff, and administrative teams, ensuring all shifts are staffed appropriately and in compliance with agency policies and state regulations.
Key Responsibilities:
- Coordinate daily and weekly schedules for home health aides, nurses, and therapists based on patient needs, staff availability, and skillset.
- Monitor staffing changes, call-outs, and time-off requests, and make adjustments as needed to maintain uninterrupted care.
- Maintain clear and timely communication with field staff, patients, and families regarding schedules and updates.
- Match caregivers with clients based on clinical needs, personality compatibility, and geographic location.
- Document all scheduling activity accurately in the agency’s scheduling platform (e.g., ClearCare, HHAeXchange, AlayaCare, etc.).
- Ensure compliance with state regulations regarding staffing ratios, licensure, and caregiver qualifications.
- Confirm that all assigned staff have current credentials, medical clearances, and training required for their cases.
- Collaborate with the clinical team to adjust schedules when client needs or acuity levels change.
- Maintain accurate and up-to-date employee and client records in the scheduling system.
- Track and report open shifts, high-priority cases, and ongoing staffing needs to the management team.
- Assist with scheduling for new patient onboarding and initial case assignments.
- Participate in a rotating on-call schedule, including evenings and weekends, to respond to urgent staffing issues or last-minute call-outs.
- Respond promptly to phone, text, and email communications during on-call shifts to ensure coverage continuity and support field staff.
Qualifications:
- High school diploma or equivalent required; associate’s or bachelor’s degree preferred.
- Minimum 1–2 years of experience in home care, healthcare staffing, or scheduling required.
- Excellent verbal and written communication skills.
- Ability to remain organized, detail-oriented, and responsive in a fast-paced remote environment.
- Proficiency with scheduling and communication software platforms (e.g., AlayaCare, Slack, Microsoft Teams, etc.).
- Working knowledge of HIPAA and patient confidentiality standards.
- Ability to remain calm under pressure and find creative solutions for staffing coverage.
- Reliable internet connection and dedicated workspace for remote work.
- Availability to work on-call evenings and weekends on a rotating basis.

Pay: $21.46 - $23.32 per hour
