JOB SUMMARY:
The Central Admissions Coordinator is part of our Central Admissions Team, within our marketing and business development department. The Central Admissions Coordinator reports directly to the Central Admissions Supervisor.
RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:
- Manages and takes ownership for admission inquiries and calls from hospital discharge planners, families, and other referral sources.
- Manages the admission process by maintaining updated bed availability and facility services information at all times.
- Manages the inquiry process professionally, timely and with appropriate follow-up.
- Effectively manages occupancy levels and census/premium mix enhancement.
- Reviews and screens all patients by comparing patient’s condition to admission criteria; evaluating and accepting or rejecting patients; referring patients and family to other programs and institutions as appropriate.
- Admits patients by completing admission and financial responsibility forms; coordinating and arranging physical, social, emotional, and support service requirements, including transportation.
- Works with facility administrators and marketing team to maintain daily bed availability and up-to-date admission capabilities grid.
- In collaboration with marketing team, markets programs and facilities by preparing and providing informational brochures and answering questions.
- Develops and maintains relationships to advise referral sources of bed availability and new products and services, and efficiently meets the daily needs of the community at large.
- Maintain the current database of existing and potential referral sources.
- Prepares monthly admissions reports by collecting, analyzing, and summarizing data and trends.
- Delivers high level of customer satisfaction, monitors and evaluates Central Admissions partnership needs.
- This position is administrative, customer relations and sales. There is no direct patient care is performed in this role.
REQUIRED QUALIFICATIONS:
- Preference will be given to those that have 1 years experience coordinating skilled nursing facility admissions.
- 2-3 years medical experience.
- Knowledgeable of nursing and medical practices and procedures, as well as laws, egulations and guidelines that pertain to nursing care facilities.
- Knowledge of Medicare/Medicaid preferred.
- Demonstrated ability to understand and promote technical information (i.e. clinical outcomes, data points).
- Active listening skills and excellent verbal and written communication skills; must be a team-player.
- Previous experience should include demonstrated customer service attitude, innovative work ethic, persuasive telephonic conversation and organizational skills.
