PHC Coordinator

Ace Primary Home care is currently seeking highly motivated individual that is responsible for managing the placement of attendants in clients' homes, addressing client concerns, and ensuring high-quality service delivery within the Personal Home Care (PHC) program. Two years of PHC experience as a coordinator a must. This role requires strong communication skills and the ability to maintain positive relationships with clients and referral sources while adhering to program regulations and quality assurance standards. Experience as a PHC Coordinator is a must.

Responsibilities

• Recruiting and placing attendants in clients' homes.
• Addressing client concerns and inquiries.
• Communicating with clients, referral sources, and doctor's offices via phone and in writing.
• Performing data entry tasks related to client and program information.
• Problem-solving to address issues that arise within the program.
• Maintaining positive working relationships with referral sources.
• Providing outstanding customer service to clients and their families.
• Monitoring and tracking program authorizations.
• Ensuring timely completion of paperwork and compliance with program regulations.

Requirements

• Two years of PHC experience as a coordinator a must.
• Excellent written and verbal communication skills.
• Positive attitude and ability to work independently.
• Compassion for working with the aged and disabled.
• Verifiable work history with good references.

Job Types: Full-time, Contract

Pay: $12.00 - $18.00 per hour DOE

Work Location: Pharr, Tx (In office)
