Responsibilities:
- Engage consumers at the point of hospitalization and participate in discharge planning.
- Provide post-discharge follow-up to support consumer transition to the community.
- Develop and implement individualized service plans to meet consumer needs.
- Maintain accurate records and complete required documentation.
Requirements:
- Bachelor's Degree in Human Services or Psychology.
- At least 2 years of experience in mental health services with adults preferred.
- Professionalism and commitment to excellence in care.
- Valid driver's license, use of an insured vehicle, and an acceptable driving record.
