Requirements:
· Licensed Advanced Practice Nurse (APN) in the state of Arizona.
· Reliable transportation and a verifiable good driving record. Must be in the Jacksonville metro area, able to work from home, and able to travel to patient homes.
· Must be self-motivated and able to perform work in accordance with defined operating procedures.
· Ability to communicate effectively both orally and in writing.
· Must be comfortable interacting with patients and care teams both telephonically and in-person.
· Bilingual in English and Spanish is preferred.
Job Type: Full-time
Pay: $140,000.00 - $150,000.00 per year
