Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, particularly for individuals requiring specialized support in geriatrics and hospital medicine. This position requires a strong understanding of medical terminology and effective patient service skills to ensure optimal outcomes for our patients.
Duties
- Conduct comprehensive assessments of patients' needs, focusing on those in a Level II Trauma Center setting.
- Develop and implement individualized care plans in collaboration with multidisciplinary teams.
- Manage cases for patients across various settings, including primary care, hospital medicine, and PICU.
- Provide education and support to patients and their families regarding treatment options and care processes.
- Monitor patient progress and adjust care plans as necessary to meet evolving needs.
- Facilitate communication between healthcare providers, patients, and families to ensure continuity of care.
- Maintain accurate documentation of patient interactions and case management activities.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- InterQual experience required
- Proven experience in case management within a hospital or clinical setting.
- Strong knowledge of geriatrics, primary care, hospital medicine, and PICU practices.
- Familiarity with medical terminology relevant to patient care.
- Excellent interpersonal skills with a focus on patient service and advocacy.
- Ability to work collaboratively within a multidisciplinary team environment.
- Strong organizational skills and attention to detail.
