Shifts Available:
- Day Shift: 8:00 AM – 4:30 PM (Rotating Weekends)
- Evening Shift: 12:00 PM – 8:30 PM (Flexible to 11:00 AM – 7:30 PM if needed, Monday – Friday, No Weekends)
Job Summary:
The RN Case Manager - Observation is responsible for managing the care progression of observation patients. This role involves reviewing cases promptly, applying InterQual criteria, and ensuring compliance with the 2-Midnight Rule. The RN Case Manager works closely with physicians to facilitate timely decisions regarding patient admission or discharge. They identify and address barriers to patient progression and escalate unresolved issues to leadership as needed.
Additionally, this role focuses on identifying patients with discharge planning needs, ensuring safe transitions of care, and coordinating resources to improve patient outcomes. The RN Case Manager collaborates with the healthcare team to optimize patient care, maintain quality, and manage resource utilization effectively.
Key Responsibilities:
- Provide case management services for inpatient and observation patients.
- Identify patients at risk for adverse outcomes during transitions of care.
- Conduct comprehensive assessments of psychosocial, medical, and discharge needs.
- Reassess clinical conditions and implement strategies to mitigate readmission risks.
- Collaborate with physicians and multidisciplinary teams to coordinate patient care and discharge plans.
- Partner with Social Services to address post-acute medical and psychosocial needs.
- Use evidence-based tools (InterQual) to evaluate and support patient progression.
- Facilitate timely referrals to third-party payers and case management programs.
- Document case management activities and ensure effective communication with healthcare teams.
- Act as a liaison between physicians, patients, families, hospital staff, and external agencies.
- Advocate for patient rights and ensure culturally competent care.
- Participate in performance improvement initiatives and compliance with regulations.
Qualifications:
- License: Active RN license (Compact license holders must obtain a state-specific license within 60 days of relocation).
- Certification: BLS (AHA) required within 30 days of hire.
- Education: Associate Degree in Nursing (ADN) required; Bachelor’s Degree in Nursing (BSN) preferred.
- Experience: Minimum of 1 year in case management with InterQual experience. Strong utilization management experience is preferred.

Compensation & Benefits:
- Salary Range: $32.91 - $45.25/hr (based on experience)
- Shift Differentials:
- Evenings: $2.00/hr
- Weekends: $2.00/hr
- Sign-On Bonus: Up to $15,000
