Job Type Full-timeDescriptionPosition Function : To support and serve youth and their families by providing and coordinating case management services, therapeutic services, crisis management and transitional services. The clinical case manager will respond to referrals and coordinate admissions for assigned programs. The clinical case manager will provide admission assessments, develop service plans, assist in setting goals and coordinating resources to stabilize behaviors and provide independent living skills to youth transitioning to permanence or independence. The job requires sensitivity to the service population's cultural and socioeconomic characteristics.Education Requirements : A Master's degree in social work, psychology, or related human services field from an accredited college or university is required with additional certification preferred. Experience Requirements : The position requires two years of cumulative experience with an undergraduate degree that includes working with youth, crisis intervention, intake and assessment, and case management. Understanding of crisis intervention and trauma informed care interventions, counseling techniques in working with adolescents and families, good verbal and written communication skills. Possess a working knowledge of basic computer skills. Functional Requirements : Requirements include providing expedient and ongoing assessment, service planning, case management, and transition planning for all assigned clients. Maintain accurate and timely documentation of services for all assigned clients. Prepare and submit all required documentation, reports, and statistical data in a timely manner. Follow policies and procedures of ACH and represent the agency in a positive and professional manner. Coordinate the collaboration of referral agencies, family members, and other support systems to meet the needs of youth. Effectively communicate with the campus staff to coordinate service plans of each youth and lead groups (life skills, team building, etc.) with youth in assigned programs. Residential programs are in operation 24 / 7. This position provides on-call support for clinical and program staff. Working Conditions : Exposure to children with behavior problems, including possible physical aggression. Exposure to illness and blood.Exposure to Confidential Information : Maintain confidentiality & follow policies related to personal records & client records.Key Expectations / Responsibilities : Programming : Provide 24-hour response to all client referrals in coordination with program leadership.Manage admissions to assigned programs as assigned through screening and assessment of all referrals and facilitation of placements.Facilitate and document admission assessment and service planning for all assigned clientsComplete all admission paperwork within required time framesProvide expedient case management and therapeutic interventions Provide skills training for assigned clientsImplement group programming to address the social and emotional needs of clients relating to achievement of individual goals.Provide case management services; coordination with TDFPS caseworkers, families, community and mainstream support resources.Document and assess progress of each assigned client Develop discharge and aftercare plans and follow-up referrals. Provide aftercare services as required. Community Relations Coordinate the collaborative relationships and services to referral agencies.Build and maintain positive working relationships with local human and youth services agencies.Represent ACH in a professional and positive manner. Documentation and Reporting Maintain up-to-date case records.Develop and document initial assessment and initial plan of serviceProvide regular case management services for each assigned clientDocumentation of case progress notes.Communication and coordination of needs for each clientCommunication of goals and service plan with the direct care teamAssist in development of program related forms and measurement tools.Developing, reviewing, and updating of service plansCompleting a discharge and after care plan for each client Work Behaviors Support and uphold the policies and procedures of ACH at all times.Maintain a positive morale.Ensure that quality services are provided and professional standards maintained.Maintain an attitude of youth and family advocacy.Work harmoniously with co-workers to generate a productive and constructive environment.Ensure that work time is utilized fully and effectively to maximize productivity.Adapt to changing assignments / priorities as needed to support youth, families, and the agency.
