POSITION SUMMARY
The Case Manager (CM) takes initiative, can multi-task, and is able to manage a large caseload. The CM provides assessments, assignments, and accountabilities to individuals in our program. The CM reports directly to the Clinical Director.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:
· Uphold all program policies and written agreements.
· Assign, and maintain complete case management meetings.
· Maintain records of Clients legal needs and requirements, understand what attention is needed in order to follow client needs.
· Respond to incoming calls.
· Oversee and document program application and intake process including documenting applicant pre-screens and arrival..
Knowledge of filing FMLA/ STD
Working with Probation Officers
Knowledge of community resourses
· Collaborate weekly with Clinical and Nursing departments on client care ensuring lines of communication between departments is clear and routine.
· Coordinate and lead weekly orientation meetings as assigned.
· Administrate all case management records and documents both as hard copies and in computer
· Provide reports, spreadsheets and statistics as requested.
· Assist in the development of computer notes to improve case management communication.
· Oversee Participant case files to ensure progress through our program to coin out.
· Perform comprehensive assessment of participant needs
· Recommend the necessary level of intervention, in collaboration with Clinical and Nursing Staff.
· Develop and regularly modify treatment service plan as directed
· Hold clients accountable to program guidelines.
· Monitor and assess participant progress towards identified goals.
· Assign workshop requirements for your assigned clients and maintain workshop or group signup sheet.
SKILLS AND ABILITIES
· Ability to define problems, collect data, establish facts, and draw valid conclusions
· Ability to work as a team member and have management, communication, organizational and interpersonal skills
· Ability to work under stressful conditions and be flexible in relation to department needs
· Knowledge of Joint Commission standards within the Department.
· Knowledge of State and Federal Statues Regarding Patient Confidentiality Laws
· Knowledge of Drug Free Workplace Policies; Workplace Violence
· Knowledge of Corporate Integrity and Compliance Program
MINIMUM QUALIFICATIONS
BA degree in related field of social work
Experience in case management.
Ability to work independently with minimal supervision.
Knowledge of case assessment and referral procedures.
Ability to work with clients who are struggling with addiction and behavioral issues.
Good interpersonal skills with ability to be respectful and courteous to clients and co-workers.
Case Manager position will be Tuesday-Saturday or Sunday-Thursday
Job Type: Full-time
Pay: $38,000.00 - $50,000.00 per year
