Job Summary : The Case Manager for our Preschool Program is responsible for working collaboratively with the Preschool Project Manager, IEP Manager and other Case Managers to ensure all children receiving services through the Preschool Department receive high quality services which meet all state, City / County and federal expectations.
Please note that this is not a remote position
Essential Duties and Responsibilities :
- Quality assurance responsibilities : internal case audits, parent surveys and review of concerns regarding cases.
- Data entry : enter, update and maintain data from various source documents, review for deficiencies or errors
- Review and analyze available reports to ensure the consistent provision of services; provide weekly update to the Program Director
- Regular monitoring of therapist productivity to ensure approved services are provided consistently; communicate any identified concerns to the appropriate Manager or Case Manager.
- Troubleshoot any issues that may arise throughout the life of a case.
- Ensure all cases meet state and county regulations
- Provide coverage and / or or assistance to the Preschool Department as needed
- Other duties as assigned by Director
