Job Overview
We are seeking a dedicated and compassionate Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the necessary support and resources throughout their treatment journey. This position requires strong communication skills, a thorough understanding of medical terminology, and experience in managed care settings. The Case Manager will work closely with patients, families, and healthcare providers to facilitate effective discharge planning and continuity of care.
Responsibilities
- Conduct comprehensive assessments of patients’ needs, including medical, psychological, and social factors.
- Develop and implement individualized care plans that address the specific needs of each patient.
- Coordinate with multidisciplinary teams within hospitals and outpatient settings to ensure seamless transitions of care.
- Provide mental health counseling as needed, offering support and guidance to patients facing emotional challenges.
- Engage in discharge planning activities to prepare patients for a successful return home or transfer to other facilities.
- Monitor patient progress and adjust care plans as necessary based on ongoing evaluations.
- Maintain accurate documentation of case management activities, including diagnostic evaluations and treatment plans.
- Educate patients and families about available resources, services, and treatment options related to their health conditions.
- Advocate for patients’ rights and needs within the healthcare system.
Qualifications
- Bachelor’s degree in Nursing, Social Work, Psychology, or a related field; Master’s degree preferred.
- Experience in case management within a hospital or managed care environment is highly desirable.
- Knowledge of medical terminology and familiarity with discharge planning processes.
- Experience in substance use disorder treatment or behavioral health
- Strong skills in diagnostic evaluation and mental health counseling are preferred.
- Excellent communication skills with the ability to build rapport with patients and their families.
- Ability to work collaboratively within a multidisciplinary team setting.
- Strong organizational skills with attention to detail.
