Job Title: Licensed Practical Nurse (LPN)

Position Summary:
The Licensed Practical Nurse (LPN) provides patient-centered nursing care to veterans within their scope of practice as defined by the Louisiana Practical Nurses Practice Act. Under the guidance of a Registered Nurse, the LPN plays an essential role in implementing health promotion and disease prevention interventions, managing both acute and chronic conditions, supporting rehabilitation efforts, and delivering compassionate end-of-life care.
This role involves maintaining clinical excellence, documenting patient care accurately, and ensuring patient safety in compliance with all regulatory standards.
Key Responsibilities:
- Support the Registered Nurse in developing and executing interdisciplinary care plans.
- Evaluate and communicate patient/resident health care needs and changes in condition.
- Administer PO (oral), IM (intramuscular), and subcutaneous medications, including patient education on medication use.
- Perform tube feedings, prepare and serve patient meal trays, and assist with feeding as necessary.
- Assist with wound care, including dressing changes and obtaining cultures.
- Collect and document vital signs (including pain), intake/output, and clinical observations in the electronic health record (EHR).
- Provide assistance with urinary and bowel elimination procedures, including catheters and colostomies.
- Perform capillary blood glucose monitoring and insert intravenous lines when appropriate.
- Participate in patient admission and discharge processes.
- Provide comfort care measures including hygiene, repositioning, bathing, and range-of-motion exercises.
Supportive & Emergency Response:
- Respond to medical emergencies in accordance with training and under RN direction.
- Provide emotional and spiritual support to patients/residents and their families.
- Immediately report any incidents of abuse or changes in patient/resident condition to nursing leadership.
Safety & Compliance:
- Maintain patient confidentiality and ensure a safe care environment.
- Comply with VA and regulatory standards including The Joint Commission, Long-Term Care Survey Program, and the Office of Inspector General.
- Ensure accurate documentation and timely completion of all assigned nursing responsibilities.
Qualifications:
- Current and unrestricted LPN license in the state of Louisiana.
- Basic Life Support (BLS) certification required.
- Minimum of 2 years of LPN experience in a clinical setting (VA or long-term care experience preferred).
- Proficient in using electronic medical record systems.

Work Schedule:
- Scheduling will be available for 12-hour shifts.
- Must be available for a minimum of 36 hours per week.

Job Types: Full-time, Contract
Pay: $25.00 - $28.00 per hour
