These are rewarding and interesting positions with the option of approximately 75% of weekly hours being done via telehealth, remotely, either from home or from the office. Our team does a lot of remote work, but still needs to be accessible, so a commute of under 35 minutes to York County is highly preferred, given our beautiful but hazardous Maine winters! Acorn & Oak strives to care for providers as well as clients, and the agency offers lower caseloads, higher pay, plenty of PTO, Sick Leave, and Paid Holidays to support this along with flexibility, autonomy, and other essential benefits.
Maine state rules require TCMs to have a BA or BS degree, and if that degree is in a field unrelated to human services, additional oversight and supervision is required unless the individual has a strong history of employment in social services. Candidates with prior TCM experience will be offered compensation that recognizes this advantage and are encouraged to apply.

Targeted Case Management is provided through Mainecare Section 13 to identified adults or children who live in the community and intended to help each individual to lead a stable, safe and healthy community life.
Case Managers conduct intake, coordinate comprehensive assessments of the individual's strengths and needs, produce an individualized support plan (ISP) to address those needs, coordinate, advocate for and develop services identified in the plan, monitor the individual's progress, and evaluate the appropriateness and effectiveness of services. The case manager works with the family to assist with the coordination and advocacy of services for the individual.
