Job Description: Children’s Targeted Case Manager at Saco River Health Services
Summary: Responds to Family referrals within the community of York County. the child and the family’s level of function,supportive services within community and school to support child needs, with Guardians around utilization of supports, in coordination of services and supports as they provide stability in child and family’s life.
Responsibilities:
- Responds to Family request for support with assessment of child and family’s developmental level, discussion around perceived need for stabilization and support.
- Assists Families in developing goals and a plan to solve problems, identify and implement supportive services to best address Family’s needs.
- Works as a supportive part of the child’s team. Works with agencies and service providers to facilitate case coordination and information sharing.
- Works with agencies/supports which are currently involved with the child/family, or are needed during treatment, including schools, mental health agencies, hospitals, welfare agencies, and law enforcement.
- Works as a supportive mandated Reporter: Refers child victims and their families to appropriate community agencies such as crisis intervention; legal advocacy; case management; counseling agencies for medical, mental, physical, social and educational needs.
- Supports during supportive service assessments with Family.
- Works with payment sources to ensure necessary services are documented correctly for funding sources and client’s reimbursement covers necessary services. Completes Authorization forms in a timely manner in order (Ten days prior to Continued Stay Review expiration) to prevent a lapse in client services.
- Maintains case records and completes appropriate, timely documentation. Documents and maintains comprehensive patient charts, including assessments, appropriate agency paperwork, service plans, and documentation of face to face visits.
- Discussed and evaluates effectiveness of care plan and progress made by clients.
- Completes other duties as assigned.
Staff Qualifications
Education: Baccalaureate Degree in social services, may be independently licensed or not, through the State of Maine. Each full time case manager at SRHS will require 4 hours of supervision a month, provided by agency.
All case managers must possess:
· Computer skills ( ie ability to word process, use an Apple product, receive/return email, log onto a secure portal, navigate the Internet, utilize a cell phone with voice mail/messaging)
· Excellent written and verbal skills
· Ability to multi-task and to work in a self-directed role
· Ability to interpret documents from other agencies or involved supports
· Ability to speak confidently for individuals in a variety of settings.
· Able to manage conflict settings with strong communication skills
· Comfortable with interface with local law enforcements, other social supports and DHHS/CPS.
· Ability to drive and navigate the community with confidence.
Physical Effort Required: Work may involve extensive standing, walking, bending, squatting, stooping, sitting, driving. Must be able to perform fine motor, auditory and visual activities or be able to compensate effectively.
Work Environment: Work may be stressful. Supporting Families at times means saying hard things within the scope of the work. Position requires the ability to communicate and relate to a variety of people in a variety of settings. Maintaining professionalism is a must. Group Supervision is to be utilized to support one another and problem solve work situations that may be conflictual or stressful. SRHS has an excellent supervision team and our new member will be fully supported in their role.
Professional Evaluation
Each staff person will receive an evaluation each year to review performance and job satisfaction. Your supervisor may elicit information regarding your work performance from peers and support staff. Other indicators on annual reviews are quality of care provided, job knowledge and pursuit, attendance, adherence to policy, independence in your role, judgement and initiative.
