Job Summary
We are seeking a Director of Case Management to oversee and optimize case management, utilization review, and discharge planning in our healthcare facility in Hickory, NC. This is a leadership role where you’ll play a crucial part in ensuring high-quality, cost-effective patient care.
What You’ll Do:
*Direct and coordinate all aspects of case management, discharge planning, and utilization review
*Ensure compliance with state, federal, and regulatory guidelines
*Collaborate with physicians, nursing teams, and social workers to ensure seamless patient transitions
*Develop and implement strategies to improve efficiency, patient outcomes, and cost management
*Lead, mentor, and develop a strong case management team
*Monitor patient care plans and advocate for quality, patient-centered services
What We’re Looking For:
✔ Active RN License in North Carolina (or Compact State License)
✔ Bachelor’s in Nursing (BSN) required; Master’s preferred
✔ Minimum 5 years of experience in case management, including leadership roles
✔ Strong knowledge of utilization review, discharge planning, and healthcare regulations
✔ Excellent leadership, communication, and problem-solving skills
