ACT Supported Living Services provides in-home supports to adults with developmental / intellectual disabilities who live in their own homes. These supports are generally for ADL's and so the main staff are not medically trained. The position of Nurse Consultant provides in-home consultation for clients and support to staff for basic medical care (medication management, after-care procedures, checking and reviewing vitals). Part of the role is providing instruction to staff when needed to better support clients. Main areas of focus tend to be blood pressure, blood glucose, and weight management, providing opinion on any developing possible sores, cellulitis, infection, etc., and suggesting for care or referring to doctor / hospital. This is not acting as a medical provider, but supporting the client and staff in best managing their health within their home.
This is a very part-time position as the hours each client eligible to receive the service are determined by the San Diego Regional Center, of whom we are a vendor. Often hours are 5-15 per month per client, and we have a handful of clients who are eligible and in need of the service. We do not determine your regular schedule, we are flexible and willing to accommodate any other work, school, etc. Personal, reliable transportation is required.
