Job Summary:
We are seeking a dedicated and experienced Community Health Worker Navigator to join our team. The ideal candidate will have 3-5 years of experience in community health, healthcare navigation, or a related field. This role involves working closely with community members, healthcare providers, and social services to ensure individuals receive the care and resources they need. The Community Health Worker (CHW) plays a crucial role in helping patients and their families navigate and access community services, telehealth, and medical resources. CHWs support providers and manage referrals through an integrated approach that includes patient engagement, scheduling, screening, care management, and community outreach. They offer social support and informal counseling, advocate for individual and community health needs, and provide services such as health education, health system navigation, community outreach, marketplace insurance enrollment support, and participation in off-site special events.
Key Responsibilities:
- Community Outreach: Build and maintain relationships with community members and healthcare providers. Conduct outreach activities to educate the community about available health services and resources.
- Healthcare Navigation: Assist individuals in navigating the healthcare system, including scheduling appointments, understanding treatment plans, and accessing necessary services.
- Needs Assessment: Conduct comprehensive assessments to identify barriers to care and develop strategies to address these barriers.
- Education and Advocacy: Provide education on health topics, promote wellness programs, and advocate for community members’ needs within the healthcare system.
- Resource Coordination: Connect individuals with social services, community resources, and support programs. Facilitate transportation and other logistical needs to ensure access to care.
- Documentation: Maintain accurate and up-to-date records of interactions with community members, including assessments, care plans, and outcomes.
- Crisis Intervention: Respond to crises and emergencies, providing immediate support and coordinating necessary interventions.
- Cultural Competence: Provide culturally competent care and support, respecting the diverse backgrounds and needs of community members.
Qualifications:
- Education: High School diploma or GED required. CHW Certification as a Community Health Worker.
- Licensure: Active and unrestricted certification in the state of Louisiana (e.g., CHW)
- Experience: 3-5 years of experience in community health, healthcare navigation, or a related field.
- Skills:
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Proficiency in electronic health records (EHR) systems.
- Knowledge of local healthcare resources and services.
- Strong organizational and time-management skills.
Working Conditions:
- Full-time position with a standard workweek.
