POSITION SUMMARY
The HIV Navigator will refer HIV positive individuals (newly and previously diagnosed) to prevention and essential support services. Will be responsible to increase access to care for persons with diagnosis of HIV access to care
The HIV, HCV, STD and TB education, counseling and testing to at-risk individuals. The goal of this interaction is to assist the client in recognizing behaviors that are placing them at risk and identifying solutions to reduce risks. Clients receiving HIV testing must be provided risk reduction and post test counseling with referral to ancillary services provided if needed. Assists individuals who test positive for HIV in accessing services. Position requires familiarity with community resources in order to conduct referrals to services including; substance abuse treatment, STD/TB Clinics, and other services. Provide HIV/AIDS/HCV education to public in traditional and non-traditional settings. Assists senior staff in developing new testing sites and collaborations with other entities.
ESSENTIAL RESPONSIBILITIES:
- Support and promote collaboration between HIV, STD, viral hepatitis, and/or TB programs through the support and provision of integrated screening activities delivered in conjunction with HIV testing.
- Collaborate with key staff of the participating facilities to plan, develop, and implement the integrated screening activities for STDs, viral hepatitis, and/or TB.
- Ensure that clients receive their test results, specially those who test positive
- Must be able to communicate with client on sensitive issues regarding substance abuse, sexuality and relationships in an understandable and non-judgmental manner.
- Must have good oral and written communication skills.
- Must be able to keep up-to-date and factual documentation.
- Ensure that clients who test positive are linked to appropriate medical care and receive timely and appropriate evaluation and treatment.

- Re-engagement to HIV Medical Care
- Partner Services Referrals – Health Department
- Medication Adherence education (to achieve Viral Suppression)
- Prevention and Essential Support Services
- Link persons with newly diagnosed HIV to HIV medical care and ART initiation immediately, but no greater than 30 days of diagnosis
- Link persons with newly diagnosed HIV to Medical Case Management services
- Facilitate access to linkage, re-engagement, and retention in medical care and refer or provide prevention and essential support services.
- Increase the availability of condoms
- Prepare confidential and non-confidential correspondence in professional style and format.
- Maintain effective filing system and information necessary according to agency and TDSHS
HIPAA/Compliance:
- Maintain privacy of all patient, employee and volunteer information and access such information only on a need to know basis for business purposes.
- Comply with all regulations regarding corporate integrity and security obligations. Report unethical; fraudulent or unlawful behavior or activity.
Qualifications
QUALIFICATIONS:
- High School Diploma or GED required
- Preferred (2) years’ experience in the human services field.
- Proficient technology skills: MS Office applications (Excel, Visio, Word, PowerPoint), Outlook, Microsoft Teams, EHR systems.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
- Must be willing to work a flex schedule that includes weekends and evenings.
- Must be willing to access clients in locations where at-risk individuals congregate.
- Must be willing to learn and practice phlebotomy (venous blood draws).
- Must have reliable transportation.
