Job Description
The Clinical Manager of Integrated Care ensures smooth transitions from hospital to home, reducing readmissions and improving health outcomes. This role involves providing direct care, educating participants, and coordinating with interdisciplinary teams (IDT) during the post-discharge period. The Clinical Manager supervises home care staff, ensures compliance with clinical standards, and delivers high-quality home care. Additionally, this role leads care transition planning and execution, collaborating with hospital discharge planners, physicians, and home care staff. The Clinical Manager also develops and implements personalized care plans to meet participants' health needs in line with organizational goals and best practices.
Responsibilities
- Ensure smooth transitions from hospital to home to reduce readmissions and improve health outcomes.
- Provide direct care and educate participants during the post-discharge period.
- Coordinate with interdisciplinary teams (IDT).
- Supervise home care staff and ensure compliance with clinical standards.
- Deliver high-quality home care.
- Lead care transition planning and execution in collaboration with hospital discharge planners, physicians, and home care staff.
- Develop and implement personalized care plans to meet participants' health needs in line with organizational goals and best practices.
Essential Skills
- Minimum of three (3) years of clinical experience and at least one (1-2) years of home care or a similar area.
- Current active and unrestricted license and registration in New York State as a Registered Nurse.
- Proficient in the use of a computer for email, word processing, research, and maintenance of participant records.
- Good organizational, observation, communication, and judgment skills.
Additional Skills & Qualifications
- Baccalaureate degree in nursing or a related field.
- Degree from an NLN accredited school of nursing, BSN preferred.
- Successful completion of an accredited Nursing Program.
- It’s a plus if they have one (1) year of experience working with a frail or elderly population or must receive appropriate training from the PACE organization upon hiring if they have less than one (1) year of experience.
- Be legally authorized to practice in the State in which the healthcare professional will perform the function.
- Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
- Takes initiative, is resourceful, proactive, creative, innovative, and independent. Is self-directed with the ability to work with or without supervision.
Work Environment
This role involves working in an office, clinic, field, and home care settings. The schedule is Monday – Friday, 8:30 AM – 5:30 PM, totaling a 37.5-hour work week. The pay range for this position is $135000.00 - $140000.00/yr.
