What are the responsibilities and job description for the Adult Day Services Specialist position at Active Generations?
SUMMARY:
The Participant Engagement Specialist for Day Break Adult Day Services is responsible for facilitating the scheduled activities of the program and assisting the participants with their needs. Assistance required includes scheduled activities, activities of daily living (ADLs), such as toileting and showers, and completing required agency documentation.
ESSENTIAL FUNCTIONS AND JOB DUTIES:
- Responsible for facilitating activity groups as scheduled and for getting participants to and from assigned activities.
- Responsible for performing personal care including toileting and showers as needed, housekeeping and dietary responsibilities.
- Ensure shift duties are completed, and service quality is maintained in accordance with Active Generations standards.
- Responsible for maintaining participant records to include monthly charting and incident reporting.
- Responsible for assisting with the participants’ nutritional needs while at Day Break, staff is to be aware of diet restrictions of each participant.
- Collaborate with the interdisciplinary team at Day Break to assess participant needs and contribute to care plans
- Communicated effectively with team members regarding participant changes and concerns.
- Demonstrate respect, dignity and confidentiality for all participants and staff.
- Adhere to professional boundaries and service excellence standards
- participate in professional development through staff meetings, in-services, and relevant educational opportunities.
ESSENTIAL QUALIFICATIONS:
- Ability to work the hours specified above.
- Must be able to perform each essential function satisfactorily.
- High School Diploma or equivalent.
- Experience working with elderly or adults with a physical or cognitive impairment is preferred.
CERTIFICATION, LICENSURE AND/OR REGISTRATIONS:
Valid South Dakota driver’s license, First Aid Certification, CPR Certification
REQUIRED PHYSICAL ABILITIES:
- While performing the duties of this job, the employee is regularly required to stand and sit; use hands to finger, handle or feel (keyboarding, helping program participants dress, lift and transfer patients); and talk or hear.
- The employee is required to move about and frequently reach with hands and arms; stoop, kneel, crouch (stoop to pick up laundry and load in machine, kneel to assist participants, reach for supplies on shelves at shoulder height or above).
- The employee is required to sit and occasionally climb or balance (balance while assisting a program participant).
- The employee must be able to view computer screens for brief periods of time.
- The employee must regularly lift and/or move about up to 10 pounds and occasionally lift and/or move about up to 30 pounds (lift and transfer patients).
- Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
REQUIRED COMMUNICATION/MATHEMATICAL/REASONING SKILLS:
- Must possess the required level of skills and ability to read, write, speak and apply mathematical concepts and sounds reasoning to successfully perform the essential duties and responsibilities of this job.
- Must be able to perform basic operations on a computer within the Windows operating system.
- This position requires problem solving skills, critical thinking skills and the ability to multi-task.
