What are the responsibilities and job description for the Outreach Specialist position at Active Generations?
The Outreach Specialist will spearhead outreach initiatives throughout South Dakota, promoting a range of CAREgiver services and sharing evidence-based practices and interventions. This role will facilitate a collaborative process that empowers families to achieve caregiver wellness, autonomy, and empowerment through advocacy, assessment, planning, consultation, education, resource management, and service facilitation.
Essential Functions:
- Assist caregivers with immediate and long-term care planning.
- Travel as needed to communities primarily in the western portion of South Dakota to provide services offered. Some additional travel may be required for training purposes.
- Develop and facilitate caregiver support groups based on the caregiver’s needs (general support, dementia, Parkinson’s, etc).
- Establish linkages with a variety of service providers that directly impact/serve caregivers.
- Implement public events, meetings, and open house sessions - general community presentations
- Provide reports on program outcomes for tracking and evaluation.
- Build and maintain collaborative relationships with community partners.
- Stay updated on local resources and care options.
- Attend professional meetings and outreach activities to educate the public about caregiver services.
- Obtain certification to facilitate evidence-based programs and maintain certification.
- Offer individual and group support and consultations as needed, including family meetings.
- Develop systems to track service activities, including visits, referrals, caregiver demographics, and satisfaction.
- Adhere to organizational policies and maintain professional judgment.
Qualifications:
- Bachelors Degree in Social Work, Sociology, or other related field preferred.
- 2-5 years of field experience working with the aging population or individuals with dementia-related disorders.
- Experience with individual and/or family caregiver consultation support.
Physical Requirements:
- While performing the duties of this job, the employee is regularly required to stand and sit; use hands to finger, handle or feel a keyboard, file papers, and take notes; and talk or hear.
- The employee is required to move about and frequently reach with hands and arms; stoop, kneel, crouch to file paperwork.
- The employee is required to sit and occasionally climb or balance to reach supplies.
- The employee must be able to view computer screens for medium periods of time. 50% or more of the time for this position is computer-related.
- The employee must regularly lift and/or move about up to 10 pounds and occasionally lift and/or move about up to 40 pounds. (supplies or shipping boxes)
- Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
