- Full support staff
- Board Certified or Board Eligible in Family Practice required
- Documentation of completion of education as Medical Doctor (MD) or Doctor of Osteopathy (DO)
- If medical education was obtained outside of the U.S., documentation from the ECFMG is required.
- Current & unrestricted Florida medical license required
- Current & clear DEA registration
- Current BLS / CPR certification
