Basic Function: Ensures person centered programming is implemented on a day-to-day basis. This includes but is not limited to assisting with the completion of implementation strategies, facilitating community inclusion, group and individualized activities and providing assistance with the functions of daily living. This position serves to protect and promote the rights and dignity of the person(s) supported through building a committed relationship based on trust and supporting that person to become an active and valued member of their community.

Principal Duties/Responsibilities:

 

  • Follows, implements, provides feedback with person-centered and behavioral outcomes.
  • Plans, performs, or assists with care or activities for persons according to their preference or needs, to include, but not limited to: cleaning, personal dishes, vacuuming, fixing bed, household maintenance, etc.), community involvement, social, recreation and leisure skills, communication skills, personal safety skills, appropriate
  • Provides opportunities for new experiences in the community.
  • Maintains professional boundaries between, team member’s families and persons
  • Serves as a mentor and coach to persons served. Provide guidance and direction while being a positive role
  • Is creative by trying new ideas that create a more positive impact for the persons we serve, team-members, and our community.
  • Maintains confidentiality in all aspects of the
  • Ensures the safety and well-being of the persons served at all
  • Assists people served with self-advocacy skills and promote
  • Transports persons supported (adhering to all safety policies and laws) as needed to meet program
  • Assists new team members by providing on-the-job
  • Exhibits flexibility to changing situations and be able to adapt as
  • Participates in required meetings.
  • Takes direction and initiate actions that will allow for cross-training at multiple sites if required.
  • Builds relationships by facilitating friendships, partnerships, and personal and professional
  • If applicable to the person, apply therapeutic intervention techniques as needed to individuals with verbal or physical
  • If applicable to the person, assist or perform personal care to include showering, bowel and bladder care, exercising, range of motion, feeding (oral or feeding tube), hair care (brushing, washing, styling, shaving) dental care (brushing, flossing, dentures) or nail clipping,
  • Displays patience when working with those we serve, guardians, and co-workers.
  • Maintains equipment and supplies necessary for the implementation of programs.
  • Accurately completes, maintains, and submits all documentation and written records as required by the department and/or Rule 50, Positive Behavior Support Plan, or other rules, regulations, and policies or procedures of the
  • Prioritizes multiple tasks according to need and the ability to multitask under
  • Looks for ways to promote and improve the quality of
  • Completes all documentation to include daily notes and documentation on goals and behavior data tracking every
  • Maintains accurate financial management and documentation of the funds of person supported.
  • Ensures routine and emergency medical services are
  • Assists in the development and implementation of Person-Centered Training
  • Assists with ensuring vehicle maintenance is
  • Other duties as assigned

General Requirements:

Environmental Conditions:

 The noise level in the work environment is usually moderate. Duties are performed in either an office or classroom environment. No unusual or adverse working conditions exist.

Qualifications:

 

Education: This position requires a high school diploma or general education equivalent. 1 to 2 years of college education is preferred. The selected candidate must be 18 years old and pass a comprehensive criminal background check.

Experience: Requires 1 year of experience working with persons with Intellectual and Developmental disabilities or 6 months of customer service experience with an interest in working with disabled individuals.

 

Additional Skills and abilities:

-This position requires good communication skills as exhibited by being able to articulate thoughts clearly both in spoken and written communication.

-This ideal candidate must possess good decision-making skills, a high-level work ethic and a dependable employment record.

-This position requires that the incumbent work well individually and within a team environment.
