Description
This is a social casework providing a full range of social services in the field of aging services. The work involves the employment of casework skills in obtaining essential information, counseling clients and family members, and aiding them to utilize all available resources. The work is performed in accordance with established regulations, policies, and procedures.
Protective services involves crisis intervention to consumers who are victims or potential victims of abuse, neglect or exploitation, or who are a danger to themselves or others.
Job Duties
- Conducting Level of Care Assessments.
- Conducts needs assessments in the community.
- Submitting assessments to supervisor within 3 days of home visit for review.
- Completing required phone calls and reassessments.
- Developing care plans with the available supports and programs to maintain independence.
- Attends and completes required Protective Services training and participates in on-call rotation. Completing Report of Needs and initiates Protective Services investigations when the Protective Services worker is unavailable, or the Care Manager is on call.
- Serves as back-up intake worker during regular business hours when primary Protective Service workers are at high volumes of investigations.
- Obtains financial information from clients and families to counsel them on receiving federal benefits that are available in the community and for future planning.
- Completes assessor certification in order to perform assessments independently, one month after hiring date. Certification includes satisfactory completion of online courses with Des Moines University and Boston University and satisfactory completion of certification exam from Pennsylvania Department of Aging.
- Performs other duties as assigned.
Required Knowledge, Skills and Abilities
Knowledge of social casework principles and methods
Knowledge of Adams County Office for Aging, Inc. (ACOFA), health care and social agencies and resources.
Knowledge of individual and group behavior and methods of working effectively with families.
Knowledge of medical terminology and abbreviations, common health problems and prescription medications.
Ability to establish and maintain effective working relationships with clients and their families, medical personnel, nursing home and county assistance office employees.
Ability to plan and organize work, prepare adequate records and reports, set priorities and maintain a caseload.
Ability to express ideas orally and in written form.
Ability to use computer to track and enter consumer data
Accessibility to a car and willingness to travel.
Minimum Education and Training
Master's degree in human services and one year of experience performing assessments
or
Bachelor's degree in human services and two year of experience performing assessments
or
Associate's degree in human services and four years of case management experience including two years performing assessments
or
Six years of case management experience, including two years performing assessments; supplemented by successful completion of 12 credit hours in sociology, social welfare, psychology, gerontology or other related behavioral sciences.
