Intake Coordinator/Specialist
We are seeking an Intake Coordinator/Specialist to join our constantly growing team. The Intake Coordinator/Specialist will be responsible for communicating with patients and referral sources in a professional manner, verifying insurance eligibility, maintaining accurate referral tracking records, and implementing new standard operating procedures in our dynamic, growing company.
Responsibilities include:
- Serving as the initial point of contact for new inbound referrals via phone, email, fax, etc.
- Receiving all inbound calls and forwarding them to the appropriate team members as needed
- Conveying any new information regarding patients in a timely manner to the appropriate team members and documenting in the EMR
- Determining if a referral is ready for clinical review or if it is deficient in any way
- Collecting, reviewing, and consolidating clinical referral documents for clinical review
- Collaborating with the clinical, staffing and marketing team on accepted, pended, or denied referrals
- Verifying insurance through provider portals (Availity, Cigna for HCP, UHC Link, etc.)
- Tracking and following new, existing, and potential readmit patients in coordination with the marketing team
- Maintaining appropriate records, files, documentation, etc.
- Assisting in the development of new processes within the intake and referral process in collaboration with the clinical, operations, and marketing teams
Required Qualifications:
· 2-3 years of healthcare experience (Home Health intake highly preferred)
· Ability to verify insurance eligibility through provider portals
· Experience with Epic systems and NaviHealth
· Strong attention to detail and ability to review and interpret clinical data
· Excellent written and verbal communication skills
· High School Diploma or equivalent (GED) required
Preferred Qualifications:
- Ability to sustain efficient, timely, and accurate work in fast-paced work environment
· Strong experience with Microsoft Office (particularly Microsoft Excel and Outlook)

The Referral Coordinator position requires working within our normal business hours of 8:00 AM - 5:00 PM. However, this position will require after-hours and weekend availability as needed.
Job Type: Full-time
Pay: $60,000.00 - $70,000.00 per year
