Position Summary:
The Counselor II provides direct therapeutic support, including conducting assessments and developing treatment plans. They work under supervision to deliver individual and group counseling for individuals with substance use disorders, ensuring adherence to both clinical and regulatory standards.
Qualifications:
· High School Diploma or equivalent Required
· LAADC 1 licensure required within 90 days of hire (TN locations only)
· CPR certification within 30 days of employment
· 2 years of verifiable sobriety if in recovery
Essential Functions:
- Conduct comprehensive Psychosocial Assessments to evaluate patient needs and develop a clinical summary.
- Develop treatment plans with specific goals, objectives, and corresponding interventions, based on the patient’s needs.
- Coordinate referrals for additional services or specialized care as necessary.
- Manage and coordinate patient services and treatment activities across multiple disciplines.
- Provide individual and group counseling sessions for patients with substance use disorders.
- Educate patients and families on recovery principles, addiction, and available community resources.
- Document patient progress, treatment interventions, and discharge summaries clearly and legibly in patient records.
- Incorporate 12-Step Programs and Principles into therapeutic processes and treatment plans.
- Maintain accurate records and documentation of all patient interactions, ensuring compliance with confidentiality and regulatory standards (HIPAA, 42 CFR Part 2).
- Support the patient’s progress by working closely with the clinical team, payers, and referral sources to provide coordinated care.
- Assist in facilitating family counseling sessions, helping patients and their families understand addiction and recovery.
- Conduct educational groups relevant to addiction treatment, including substance use education, relapse prevention, and recovery skills.
- Work in a cooperative manner with co-workers, managers, clients, and prospective clients in a professional and collaborative environment.
- Remain alert at all times, ensuring safe job performance.
- CPR certification required within the first 30 days of employment.
- Regular attendance and active participation in team meetings and ongoing training.
- Perform other duties as assigned by the Clinical Manager.
