QUALIFICATIONS/EXPERIENCE
- Licensed in the State of Kentucky as an Advanced Registered Nurse Practitioner (APRN)
- Three or more years of experience with family practice or other primary care setting
- Enrolled in the Kentucky Medicaid system
- Current DATA 2000 waiver required

GENERAL DUTIES
- Provides comprehensive health assessments for clients referred for health care by primary care team staff via telehealth or in person if practical.
- Provides both comprehensive behavioral health and medical assessments to clients within 24 hours of intake when such assessments are deemed appropriate by the intake department.
- Conduct therapy, medication assessments, and crisis response for new and existing clients. Develops individual treatment plans based upon assessment data, and provide therapeutic interventions as necessary including the provision of on-going medication management services.
- Provide regularly scheduled (usually weekly) caseload consultation to treatment team. These consultations focus primarily on clients who are new to treatment or who are not improving as expected and are typically done by telephone, telehealth, or in person.
- In case of an emergency, the Nurse Practitioner shall refer person served/client to a specialist or emergency room for further treatment. Nurse Practitioner will maintain general health checkups until issue is resolved, contained, or stabilized.
- Maintain professional cell phone account and provide occasional telephonic consultation to primary care providers (PCPs) as needed, focusing on clients in the CMs caseload. Respond to telephone calls from primary care providers and CMs within one business day. Respond to urgent telephone calls within one hour, if available.
- Maintain documentation of services including treatment plans, progress notes,
- medication informed consent, quarterly treatment plan reviews, and other required information in compliance with ARC policies and compliance standards.
- Maintain professional email accounts for contact during usual business hours. Check professional email account daily and respond to email questions/consultations within two business days, sooner if urgent.
- Suggest treatment plan changes, including medication recommendations, for clients who are not improving as expected.
- Use a population-focused registry to document recommendations for treatment and/or referrals within 24 hours of consulting so that they can be easily shared with PCPs and other treating providers.
- When possible, visit each ARC center at least once per year to meet center providers and discuss ongoing collaboration.
- Bring policy/personnel issues to supervision as they arise.
- Ability to interact with a multidisciplinary team to assist in facets of the recovery program relating to physical health.
- Have the ability to communicate with the Case Manager and assist in ensuring the safety of the persons served/client.
