Pay Range : $17.50 - $19.00

RESPONSIBILITIES & DUTIES
- Maintain positive communications and relationships with his / her supervisor, clients, employers, involved members of families and other individuals in the community.
- Participate in the evaluation of the client through completion of task analysis and other requested assessment devices.
- Provides logs, reports, and client evaluations in a timely, complete, and accurate manner as required.
- Ensure that information in client record is current and accurate.
- Assist in the design and implementation of individual training and support programs for the client(s).
- Provide on-site direct service to client at their employment and through pre-employment and employment training where and when needed
- Transport clients to appointments, community activities, or other locations as needed, ensuring safety and timeliness during travel.
- Provide direct service as needed and required
- Perform all duties of a direct service provider as assigned.
