Adroit Care Rehabilitation and Nursing Center is seeking an experienced and compassionate Director of Social Services to join our team! If you're dedicated to improving the lives of residents in a supportive and collaborative environment, we want to hear from you!
Key Responsibilities:
- Conduct interviews with residents and families to gather comprehensive social histories.
- Develop and plan social service programs in partnership with residents and families.
- Address social and environmental challenges by identifying solutions, coordinating financial assistance, facilitating discharge planning, and working with community organizations for specialized services.
- Collaborate with administration, medical and nursing staff, and other departments to design and implement effective social service programs and activities.
- Actively participate in facility surveys and inspections by authorized government agencies as needed.
- Advocate for residents by communicating their social, psychological, and emotional needs to the care team, including medical staff and attending physicians.
Requirements:
- Minimum of 3 years of experience in social services, preferably within a skilled nursing facility (SNF).
- Strong interpersonal, communication, and problem-solving skills.
