Job Description:
Essential Job Functions:
Assessing the presenting and underlying needs of identified clients
by utilizing appropriate tools such as the ANSA, GAD 7, PHQ 9, and Recovery Capital Scale and collecting information from the client, members of the treatment team and natural supports.
Developing clear and measureable plans to increase the positive health outcomes of clients
by collaborating with the consumer, consumer’s natural supports and members of the treatment team which promote the importance of social support and good lifestyle choices.
Assisting consumers to minimize the negative impacts of chronic health conditions on daily living
by providing the consumer with education, support and technology to monitor and manage symptoms and to problem solve identified health issues.
Promoting the health care of the client
by working with the consumer to build the skills necessary to independently arrange and participate in medical appointments, by linking the consumer to other programs, services and agencies and by advocating on behalf of the consumer with medical/service providers and natural supports.
Serving as the point of contact for consumer information
by timely and accurately communicating information permitted by the client to the treatment team and individuals identified by the consumer as well as ensuring that the consumer is aware of information provided by the treatment team and identified individuals.
Providing comprehensive transitional care services
by ensuring that the needs and desires of the consumer are addressed as part of a plan to transition consumer care to a different provider, level of care, living environment or other case discharge.

Knowledge, Skills, and Abilities:
Knowledge and competencies with harm reduction, motivational interviewing and relevant treatment modalities that are being used in the field.
Knowledge of culture and values in the local community.
Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
Ability to give full attention to what other people are saying, taking time to understand the points being made.
Ability to communicate information and ideas in speaking so others will understand.
Proficient in Google platform.
Excellent verbal and written communication skills.
Supervisory Requirements:

Employment Requirements:
Must provide evidence that incumbent is free of infectious and contagious disease prior to beginning employment and annually thereafter.
TB– Expected for all direct care staff.
Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.
Completion of New Hire Orientation at the beginning of employment.
All training requirements including Relias at the beginning of employment and annually thereafter.
Current driver’s license, acceptable driving record and current auto insurance.
Achievement of the following training.
Community Health Worker Certification or able to obtain within 6 months of employment
Trauma-informed training, to better understand/address foster care experiences.
Training on common behavioral health conditions
Training on suicide prevention and substance use.
First Aid certification within three (3) months of beginning employment and upon renewal date.
Obtain CPR certification within three (3) months of beginning employment and upon renewal date.
Behavioral Management training completed within thirty (30) days of employment or first available class after start date:
CPI for Residential programs
Peaceful Intervention or Verbal De-escalation for Outpatient programs
Completion of “Direct Service Expectations” within 30 Days of employment.
Extensive knowledge of the policies, procedures, and regulations of the program to which the employee is assigned.
Valid Driver’s License in state of residence
Physical Requirements:
ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up the 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
