What are the responsibilities and job description for the LPN (Licensed Practical Nurse) position at Adult Pediatric Urology & Urogynecology?
Plan and provide professional nursing assessment for patients at Adult Pediatric Urology & Urogynecology clinics in accordance with Physician or Provider orders and established policies / procedures. The LPN will take phone calls from patients. The LPN will provide professional nursing assessments, prioritize treatments according to the urgency of need and initiate the appropriate care. The LPN is responsible to ensure that all patient calls and concerns they receive are attended to in a timely manner.

1) Collects subjective and objective data over the telephone. Communicates data to Physician or Provider in a timely fashion and carries out their orders efficiently and timely. Documents in electronic medical record any conversations, changes in patient’s conditions, medication or prescriptions received, and plan of care in concise and logical manner. Responsible for gathering all records and results as needed for patient appointments that are added on for same day scheduling.
2) Provides verbal and written education to the patient regarding their condition and documents such teaching in the patient’s medical chart. Takes all hospital consults and Physician to Physician calls.
3) Consistently acts as a patient advocate for Adult Pediatric Urology & Urogynecology. Therefore, will seek out appropriate guidance and direction in the performance of responsibilities and duties. Demonstrates good judgment, and reasoning skills. Demonstrates a respectful rapport with nursing staff, medical staff, other departments, and patients.
4) Performs medication prior authorizations, refill requests, patient requested paperwork, review of imaging, labs and pathology reports.
5) Acts as a liaison between the provider and the patient to relay results and recommendations.
6) Must work as a team member and perform actions consistent with established Adult & Pediatric Urology & Urogynecology policies and procedures including standards for safety, attendance, punctuality, and personal appearance. Perform other duties as assigned.
