Responsibility Summary:
- Schedule and match caregivers according to client needs and care plans.
- Manage and update client and caregiver schedules in scheduling software.
- Provide effective communication and collaboration between caregivers, clients, and Geriatric Care Managers (GCM).
- Assist with the recruitment, onboarding, training, and retention of caregivers.
- Participate in after-hours on-call support rotation.
- Promote the agency's core values: HEART, ADVOCACY, and POSITIVE IMPACT in all aspects of the role.
Responsibility Standards:
1. Scheduling Caregivers
a. Assign appropriate caregivers based on client needs and care plans.
b. Ensure accurate and up-to-date scheduling in the agency’s scheduling software.
c. Communicate scheduling updates and changes to clients, responsible parties, and caregivers promptly.
d. Ensure caregiver schedules align with the client's care needs and preferences.
2. Communication and Collaboration
a. Act as a liaison between clients/responsible parties, caregivers, and the Geriatric Care Managers to ensure effective care delivery.
b. Provide consistent support to caregivers by answering questions and providing clarification regarding care plans.
c. Ensure clear communication about schedule changes, client needs, and updates.
3. Recruitment, Onboarding, and Retention
a. Assist with the recruitment and onboarding process for new caregivers.
b. Participate in the retention efforts by fostering a positive work environment and supporting caregiver development.
c. Participate in new-hire orientations and staff training as needed.

5. Telephone Back-Up Support

Qualifications:·
Preferred Education: Associate's degree or higher (High School Diploma required).·
Experience: 1-3 years of experience in home care, elder services, or scheduling.·
Skills: Strong interpersonal and communication skills.
- Excellent problem-solving abilities.
- Attention to detail and organization.
- Ability to pass background checks, including drug screening.
