Position Summary
The Specialized Care Connection Coordinator is responsible for supporting the Care Connection team with facilitating enrollment into care management services at ACA/NY for people who fall into the special populations category. The Specialized Care Connection Coordinator is responsible for providing support to eligible members and ensuring successful enrollment. This position is responsible for ensuring compliance and quality standards are met, as well as to upholding agency policy and practices.
ACA/NY is a 501(c)(3) not for profit organization that has been designated as a Care Coordination Organization/Health Home (CCO/HH) by New York State. ACA/NY is dedicated to meeting the needs of people with Intellectual and Developmental Disabilities by providing comprehensive care management and coordination of service. ACA/NY supports 25,000 people in its program with support services spanning New York City, Long Island, and the Lower Hudson Valley.
Duties and Responsibilities
- Works in conjunction with the Care Connection Specialist with Parental Deeming members including discussing and explaining services and the process. Complete the RSA and justification, complete an in-process Life Plan and any other necessary documentation required by OPWDD.
- Primarily responsible for the enrollment activities of members who are identified as falling into a special Special Population’s categories as per ACA/NY policy and procedure such as Children’s Waiver, crisis situations, and Ineligible settings including but not limited to, Residential School Transitions, ICF Conversions, Hospital, Psychiatric hospitals, Forensic Settings.
- Responsible for attending transition meetings and completing necessary documentation needed to facilitate the transition.
- Complete and submit the Certified Residential Opportunities (CRO) application when necessary.
- Facilitate communication with Care Connection Specialist and Medicaid Eligibility department.
- Complete all necessary paperwork related to enrollment, transfer, and discharge to/from the program.
- Work with supervisors to support capacity management and to identify current availability by region.
- Provide information to supervisor for enrollment trend reporting.
- Participate in planning activities with the Care Management team and share recommendations for ensuring seamless enrollment.
- Form and maintain working relationships with all OPWDD Regional Offices, with the goal of ensuring that ACA/NY is positioned as a responsive CCO when a referral is needed.
- Provide referral information for those who require a different service model.
- Attend department/team meetings, trainings, supervisions, etc. as scheduled and in accordance with agency practice and policy. Complete all required trainings.
- Travel throughout the designated service area to meet with people interested in receiving services as needed.
- Maintain confidentiality.
- Perform other duties as assigned.
Qualifications
- A Bachelor of Arts or Science degree plus two years of relevant experience; OR a Master’s degree with one year relevant experience.
- An additional one-year of experience completing intake and enrollment for specialized services within OPWDD supports and services required.
- Qualified Intellectual Disabilities Professional (QIDP), preferred.
- Demonstrated knowledge regarding access to member benefits (e.g. Medicaid), preferred.
- Proficiency with the use of excel and other software products used for tracking the flow of information.
- Experience using OPWDD’s CHOICES system, a plus.
- Absolute sense of integrity and personal commitment to serving people with I/DD and their families.
- Excellent interpersonal, communication organization and demonstrated ability to manage multiple tasks simultaneously.
- Ability to work autonomously.
- Demonstrate professionalism, respect, and ability to work in a team environment.
