What are the responsibilities and job description for the Specialized Care Coordinator position at Summit Home Care?
Summit Home Care is looking for a Specialized Care Coordinator to join our intake department. The Specialized Care Coordinator is responsible for communicating with patients and government entities, processing referrals, which include accurate data entry and active follow ups. Providing support to account managers to ensure referrals and referral sources are serviced adequately is an essential part of the job. If you are well-versed in Medicaid and MLTC eligibility, including ePaces, CFEEC transfer forms, Maximus procedures, and MLTC enrollment that is a major plus. NHTD, TBI and HRA experience is a plus.
Position Requirements
- Understanding of CHHA, MCO, MLTC and related insurance eligibility requirements
- Must speak FLUENT ENGLISH and FLUENT SPANISH. Really, if you don't speak BOTH FLUENT ENGLISH and FLUENT SPANISH, DO NOT APPLY. We aren't going to make an exception and nothing on your resume is going to impress us enough to change that.
- Home Care Intake Experience: 1 year
- Must have good writing skills
- Strong time management skills
- Compassionate and caring
- Must be organized, have problem solving skills, and strong follow up engagement
