What are the responsibilities and job description for the Outpatient Therapist position at Advance Community Health?
Position Summary
The primary purpose of the Outpatient Therapist is to provide individual/ family therapy to the growing behavioral health department within Advance Community Health. The Outpatient Therapist will work in collaboration within a multidisciplinary team to address the various medical and social needs of patients within the organization. This position provides consultation to other departments at Advance Community Health by sharing best practice recommendations, providing auditing of clinical records, clinical policy review and training. We are looking for clinicians that are excited and experienced in evidenced based practices to support the growing demand of behavioral health referrals.
Essential Duties and Responsibilities
- Provides individual outpatient therapy to population served by Advance Community Health.
- Contacts patients within 24 hours of assignment to schedule initial appointment.
- Engages patients in follow up calls for service needs within 24 hours.
- Documents all clinical contacts in a timely manner using EHR (Epic).
- Collaborates with prescribing Psychiatrist to ensure continuity of care for patients.
- Seeks consultation regarding high-risk cases with attending Psychiatrist and Behavioral Health Director to ensure patient is receiving appropriate level of care.
- Provides crisis intervention services to patients on an on-call basis.
- Meets weekly productivity requirements.
- Collaborates with patients’ multidisciplinary team to communicate clinical concerns and to ensure the highest standard of care is being provided.
- Completes initial treatment plans.
- Adequately addresses frequency of patient no-shows to encourage attendance in therapy (i.e. documenting attempted contacts and/or sending engagement letters).
- Completes annual assessments and clinical updates when appropriate.
- Completes necessary discharge documentation when appropriate.
- Attends monthly provider meetings/supervision for case consultation and administrative updates

- Attends Continuing Education Trainings per licensing Board requirements.
- Demonstrates cultural competence and understanding
- Engages in community outreach as needed
- Develop relationship with Community Stakeholders
Supervisory Responsibility
- None
Education and Experience
- Minimum of 1 year post graduate experience in clinical outpatient setting
- Fully licensed to practice in North Carolina with LCSW, LCMHC or LMFT
- Training/certification in evidenced based treatments including but not limited to (CBT, Trauma, etc)
- Experience working with a Medicaid population is encouraged
- Experience with working with substance use population preferred
