HYBRID POSITION - WORK FROM HOME SEVERAL DAYS A WEEK (Hybrid Position - Middle Tennessee)
AdvanceCare Health Services, LLC is seeking a compassionate and dedicated Program Assistant to train for the position of Program Manager within our DDA Department. This role is essential for ensuring the well-being and independence of the individuals we serve by overseeing their care and maintaining compliance with state and agency requirements. This is a hybrid role, combining work-from-home flexibility with on-site responsibilities at our Hermitage, TN office as needed.
Applicants must have a valid driver’s license, reliable transportation with an excellent driving record and high-speed internet.
Key Responsibilities:
- Record Management: Ensure all visits, staffing plans, and documentation are accurate, up-to-date, and comply with state and agency standards.
- Audits and Compliance: Conduct site visits and audits on schedule; monitor required drills and community visits, following up as necessary.
- Training Facilitation: Provide pre-service and in-service training to staff as needed.
- Stakeholder Collaboration: Communicate effectively with families, providers, and other agencies on a monthly basis.
- Incident Management: Assist with incident management and corrective action committees, ensuring timely and accurate resolution.
- Policy Enforcement: Enforce company policies and procedures, maintaining full compliance.
- Scheduling and Reporting: Develop and manage schedules for annual drills, appointments, and meetings; provide reports to the Director as required.
- File Maintenance: Maintain and update client files, records, plans, and lists in accordance with company policies and procedures.
- Flexible Availability: Work a flexible schedule, including extended hours and overtime when necessary.
- Additional Duties: Perform other duties as assigned by the Director or as departmental needs arise.

- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) and/or business management software.
- Effective written and verbal communication skills at all organizational levels.
- Flexibility to meet the demands of the role.
Work Environment: This hybrid position allows for several work-from-home days per week, with in-office attendance as departmental needs require.
Minimum Requirements:
- High school diploma or GED equivalent; caregiving and office experience preferred.
- Previous experience in healthcare or a corporate environment is a plus.
- Satisfactory criminal background check.
- Flexibility to adapt and learn in a fast-paced environment.
