Position Summary
We are seeking a motivated and detail-oriented Case Coordinator to join our team. The Case Coordinator plays a pivotal role in supervising Homemaker-Home Health Aides, acting as a liaison with contracting referral agencies, conducting intake assessments, and coordinating personnel placements.
Key Responsibilities
- Provide telephone supervision to Homemaker-Home Health Aides as needed.
- Assign staff to cases and outline specific duties in collaboration with Field Supervisors, families, and referral agencies.
- Conduct intake assessments and maintain accurate client records.
- Serve as the primary point of communication for contract agencies.
- Address and resolve client, family, or agency concerns.
- Manage the placement and scheduling of Homemaker-Home Health Aides, ensuring seamless service delivery.
- Participate in performance evaluations and training sessions for Homemaker-Home Health Aides.
- Ensure accurate documentation, including billing and scheduling records.
Qualifications
- Strong verbal and written communication skills.
- Proven leadership abilities.
- In-depth understanding of the supervision process.
- Ability to effectively manage complaints and problem situations.
- At least 1 year of experience in human services.
- A Bachelor’s degree in Arts/Science from an accredited institution is strongly preferred.
