Job Description
We are seeking a highly skilled RN - Hospice Case Manager to join our team at Advanced Hospice. This is a challenging and rewarding role that requires strong communication, leadership, and patient care skills.
The ideal candidate will have at least one year of experience practicing as a Registered Nurse (RN) in a hospice or similar setting. The ability to work closely with the IDG team, including spiritual care coordinators, social workers, and medical directors, is essential to ensuring proper coordination and needs for patients and families.
The successful candidate will be responsible for overseeing the implementation and ongoing assessment of patient plans of care, working closely with the IDG team to ensure accurate evaluations and comprehensive care planning. They will also communicate patient conditions and collaborate with providers to deliver high-quality care.
In addition to these responsibilities, the RN - Hospice Case Manager will provide supervision of aides and nurses, participate in continued education programs to become certified in Palliative/Hospice care, and work flexible schedules as needed.
