Full Time Home Health RN Case Manager

Job Title/Position: Registered Nurse (RN)/Case Manager - Full-Time

Reports To: Director of Nursing

PAY: $75,000 - $85,000 based on experience mileage reimbursement, iPads with company paid cellular data, Insurance (health, dental & vision) after 30 days of employment. 401k with discretionary company match.

BRIEF JOB DESCRIPTION:

The registered nurse plans, organizes, and directs home care services emphasizing health education/experience. The RN Case Manager is responsible for traveling to a patient’s home to administer the services dictated by their physician and At Home Quality Care while helping patients maintain their independence and quality of life. The full-time RN case manager will be responsible for maintaining a case load of 25 weekly visits or 30 weekly points equivalency.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:

Patient Care:

• Assess and chart observations of the patient's condition and needs at each home visit.
• Complete evaluation tasks, including medication reviews and vital signs.
• Initiate the Plan of Care and makes necessary revisions as patient status and needs change.
• Provide education to patients and family members on proper home health procedures and strategies.
• Develop a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive and rehabilitative nursing actions.
• Administer medications and treatments as prescribed by the physician.
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
• Identifies discharge planning needs as part of the care plan. Prepares patient and family for discharge.
• Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
• Coordinate with physicians, physical therapists, occupational therapists, MSWs and other individuals in the patient care plan. Perform a supervisory visit with CNA (when part of the care plan) once every two (2) weeks.
  
  
  

Communication:

• Prepares clinical notes and updates the primary physician when necessary.
• Communicate with the physician and office regarding the patient's needs and report any changes in the patient's condition; obtain/receives physician's orders as required.
• Communicate with community health-related persons to coordinate the care plan.
• Reports with home office on any updates, concerns and issues regarding the patient.
  
  
  

POSITION QUALIFICATIONS:

• Graduate of an accredited school of nursing.
• Current licensure in state of Illinois and current CPR certification.
• Management experience not required. Responsible for supervising home health aides.
• Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, nursing skills per competency checklist.
• Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.
• Must be a licensed driver with an automobile insured under the state of IL. Automobile is in good working order.
