Job Title: Home Care Coordinator III
Reports to: Executive Director
Business Director
Primary Responsibilities:
Provide communication to clinics, hospitals, patients, and their caregivers, to deliver a seamless care
process. Supporting clinicians with medication verification, plan of care coordination, obtaining verbal
orders for services needed, and other support as requested. Collaborates with supervisor to provide the
highest quality of care for our patients.
Specific Job Duties - includes, but not limited to:
- Make calls to physician offices to assist in obtaining verbal orders.
- Receive verbal orders and update patient chart accordingly.
- Prepare medication profiles prior to admission visits.
- Order supplies as requested.
- Take, and assign referrals to HCC team members.
- Coordinate and obtain patient face-to-face visits as necessary.
- Schedule patients as assigned.
- Process referrals as assigned.
- Accurately build patient medical record.
- Shadow and reverse-shadow new team members for training purposes.
- Successfully complete patient satisfaction calls.
- Assist in on-call schedule planning.
- Assist in on-call coverage during non-office hours.
- Attend on-the-job and in-service training.
- Attend clinical and all other meetings accordingly.
- All other duties and tasks as assigned.
Qualifications
Experience:
- 2 years of medical office experience and/or direct patient care.
- Ability to multi-task in a fast-paced environment.
- Ability to work and communicate professionally with patients, medical partners, employees, and
- supervisors.
- Ability to work independently and as part of a team.
Required:
- Current Minnesota PTA/LPN/COTA license in good standing.
- Knowledge of medical terminology
- Good judgment and decision-making skills.
- Strong attention to detail.
- Technologically proficient and driven by accuracy.
- Act as a role model of Advanced Medical Home Care’s values and mission.
- Regular attendance and flexibility with scheduling necessary.
Working Conditions:
- Ability to sit, stand, type, see, hear, communicate verbally and in writing.
- Ability to complete job tasks within 32-40 hours per week as stated in offer.
Salary : $64,480
