Case Manager Job Description

Job Summary:
AdvancedRM Case Management is seeking Registered Nurses (RNs), Occupational Therapists (OTs), and Social Workers (SWs) to provide essential case management services for individuals recovering from serious injuries. As a Case Manager, you will play a critical role in guiding clients through the medical system, coordinating care, managing appointments, and providing education. Your expertise will make the recovery process easier and more effective for those in need.

Key Responsibilities:

Patient Case Management

  • Assist auto injury clients with their medical, emotional, and financial needs throughout recovery.
  • Ensure clients are connected with appropriate medical providers and coordinate care effectively.
  • Facilitate referrals and help clients navigate the care system.
  • Attend client appointments and collaborate with interdisciplinary teams to discuss treatment plans and progress.
  • Work with physicians, physical therapists, occupational therapists, psychologists, and home service providers to coordinate comprehensive care.
  • Ability to travel 50% and attend in-person client appointments, assessments, and case-related meetings as needed.

Coordination of Medical Services

  • Serve as the primary point of communication between healthcare providers, ensuring clients receive timely tests, treatments, and interventions.
  • Help schedule and coordinate medical appointments, diagnostic tests, therapy sessions, and follow-up visits.
  • Ensure that clients adhere to their treatment plans and receive the necessary medical attention.
  • Educate clients and their families on injury recovery, treatment options, and rehabilitation processes.

Patient Advocacy

  • Advocate for clients to ensure they have access to the care and services they need.
  • Provide emotional support and education to clients and their families, addressing any concerns about the recovery process or claims.
  • Monitor recovery progress and identify any barriers, suggesting solutions where necessary.
  • Ensure clients feel supported and valued throughout their recovery journey.

Claims and Documentation Support

  • Maintain up-to-date, accurate documentation of all case management activities and patient progress.
  • Document communications with healthcare providers to ensure accurate reporting and support claims processes.

Qualifications & Skills:

  • Education & Licensure:
  • Active Registered Nurse (RN) license, Occupational Therapist (OT) license, or Social Worker (SW) license in Michigan required.
  • Certification in Case Management (CCM) or Medical Case Management preferred but not required.
  • Experience:
  • Minimum of 3 years of clinical experience in nursing, occupational therapy, or social work.
  • Experience in coordinating patient care is preferred.
  • Skills & Competencies:
  • Strong critical thinking and problem-solving abilities.
  • Excellent communication and interpersonal skills, with a compassionate approach to patient care.
  • Detail-oriented with strong organizational and time management capabilities.
  • Ability to work independently and collaborate effectively with multidisciplinary teams.
  • Proficient in using case management software and electronic health records (EHR).

Work Environment:

  • Hybrid work environment, combining office-based tasks with on-site visits to clients, medical facilities, and rehabilitation centers.
  • Ability to travel as required to serve clients and attend appointments.

Schedule:

This position follows standard business hours but may require flexibility based on client needs, provider availability, and travel requirements.
