We're looking for a Senior Care Clinician to join our Advantmed team. As a Senior Care Clinician, you'll conduct in-home wellness risk adjustment assessments for Medicare members and other at-risk populations.

Job Description
- Conduct annual in-home wellness visits and risk adjustment health assessments on populations with chronic conditions
- Provide individualized quality care to elderly populations in their homes
- Deliver patient health education on topics like pain management, medication adherence, and healthy living
- Build relationships of trust with members through exceptional communication and empathy
- Assist in closing quality care gaps, such as screenings and lab tests
Required Skills and Qualifications
- Valid unencumbered NP license for the state where you'll be working
- Previous in-home risk assessment experience preferred
- Previous 1099 (PRN) experience is preferred but not required
- 3 years of patient care experience, preferably in primary care, adult-geriatric, or EMR settings
- Ability to work independently and within our EMR system
- Access to reliable transportation for travel to member homes within a designated area
- Bilingual skills are a plus
