What are the responsibilities and job description for the Service Coordinator position at Advent Christian Village?
- Assess new members at admission for service needs & preferences
- Develop service plan & change as needed periodically
- Assist member in transition to ACV community & monitor regularly for effectiveness of service plan
- Accurately record member profiles and maintain accurate & timely records
- Maintain contact with family members and other significant individual(s) on behalf of the member
- Bachelor degree from fully accredited university in relevant senior social services field required**
- Minimum of two years of relevant experience strongly preferred
- Must work well with senior adults**
- Valid Florida driver’s license required**
