We are seeking a Care Management Senior Manager to join our team! As a key member of our Care Management department, you will be responsible for the daily operations and supervision of team members.
About the Role:
- Coordinate care activities to ensure efficient and effective discharge planning.
- Develop and implement strategies to improve operational efficiency and patient outcomes.
- Collaborate with interdisciplinary teams to achieve hospital metrics and regulatory compliance.
- Ensure accurate and timely documentation of care management activities.
The Ideal Candidate:
- Minimum 3 years of direct Care Management/Utilization Management experience in an acute care setting.
- 2 years of Manager-level (or above) experience in Care Management Leadership.
- RN or MSW degree required.
- Strong leadership and communication skills.
