**Job Summary**
We are seeking a highly experienced Care Management Senior Manager to oversee the daily operations of our Care Management department. The ideal candidate will have a strong background in healthcare management and a passion for improving patient outcomes.
Key Responsibilities:
- Coordinate care delivery to ensure seamless transitions between hospital and post-acute settings
- Implement Corporate Care Management strategies to improve operations and outcomes
- Collaborate with the Interdisciplinary Team to sustain process improvement and attain hospital metrics
- Ensure compliance with regulatory standards and documentation guidelines
- Develop and implement strategies to improve customer experience and reduce readmissions
Requirements:
- Registered Nurse (BSN) or Social Worker (BSW)
- Masters in related field
- 3 years of direct Care Management/Utilization Management experience in an acute care setting
- 2 years of Manager level (or above) experience in Care Management Leadership
- LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED: RN, Clinical Social Worker License, Registered Nurse
