- Conduct assessments and develop individualized treatment plans for patients based on their physical needs and abilities
- Provide in-home physical therapy services to patients in accordance with their treatment plans
- Continuously assess and adjust treatment plans as needed to ensure progress towards patient goals
- Communicate regularly with patients, family members, and other healthcare professionals to coordinate care and ensure effective treatment
- Document all patient interactions, assessments, and treatment plans according to AdvisaCare standards
- Adhere to all state and federal laws and regulations related to physical therapy practice
- Maintain accurate records and reports of patient progress and treatment outcomes
- Current licensure as a Physical Therapist in the state of Michigan
- A minimum of one year of PT experience, ideally in a home health care setting, is preferred
- Strong understanding of physical therapy best practices and treatment methodologies
- Excellent communication and interpersonal skills, with the ability to connect with patients and their families
- Ability to work independently and manage a caseload of patients effectively
- Must have a valid driver's license and reliable transportation
